 Criticare/Chadds Ford
Alternacare provides a full range of alternative health care
services and assistance to the elderly, the disabled, and to
individuals recovering from illness, injury or surgery to
clients residing in Delaware and Chester Counties within the
State of Pennsylvania. Our In-Home services allow you to be
cared for in the comfort of your own home, secure and close to
family and friends. We can even provide 24-hour live-in
services allowing you to remain at home. We also provide
personalized care to individuals living in assisted living and
personal care units.
Each of
our client's needs are evaluated by our professional staff.
Together with you, your family member(s), and our staff we
develop a Plan of Care that best meets your needs and desires.
Services generally fall into these categories:
- Personal Care Services: Personal care services include
activities of daily living such as assistance with bathing,
grooming, skin care, nail care, shaving and dressing. Also,
assistance with ambulating, transferring, toileting. Meal
preparation and assistance with feeding is also included in
this category of service.
- Home Support Services: Home support services include
making the bed, changing the bed linens, light laundry,
light dusting/vacuuming, cleaning the bathroom and kitchen.
- Respite/Companion Services: For those individuals who
are caring for a loved one on a full time basis but need the
time to tend to their personal needs and also to allow for
periods of rest we provide respite services. These services
relieve the primary caregiver so that they may tend to their
personal needs.
- Transport/Escort Services: This service assists clients
in getting to doctor's appointments, to the food store,
church, etc. and provides the necessary support and
assistance in the process.
- Companionship: This service includes engaging the client
in conversation of their interest, participating in hobbies
such as reading, crafts, and engaging them in physical
activity such as taking a walk.
For many
of our In-Home Service clients we provide a combination of the
services described above. However, priority in every plan of
care we develop always includes SAFETY. Yours or your family
member's safety and well being is of paramount importance to
us! Whatever your health care requirements — long term, short
term, live-in, occasional Criticare/Chadds Ford Alternacare
will be there for you!

 Criticare/Chadds Ford Alternacare
has a full complement of health care staff ready and available
to address your special needs. They range from licensed
professional nurses, RN's and LPN,s to Home Health Aides and
Companions. Professional nurses under the direction of your
doctor, can perform physical assessments, administer
medications, and -when required-undertake additional advanced
care treatments. Our home health aides and companions provide
many of the services as described under Personal Care, Home
Support and Respite/Companion services above. We are there to
assist you in keeping and maintaining your independence while
insuring your safety!

Although
you may recognize the need for our services, many of our
clients struggle with the issue of bringing home health
aides/companions into their homes. For many it marks a
timeline when you must recognize that you need help to stay
independent and safe in your home environment. For others,
they feel uncomfortable bringing a stranger into the privacy
of their home. You are not alone! The owners of
Criticare/Chadds Ford Alternacare recognize that for many
individuals this is a significant issue! They also recognize
that the elderly or infirm are vulnerable to unscrupulous
individuals who see the elderly/infirm as easy prey. Therefore
we are committed to doing everything possible to provide fully
qualified and screened workers into your home
environment.
 All home health aides/companions
are carefully screened, interviewed, and complete a lengthy
application process that includes two inservice programs.
Further, they must have appropriate references and are put
through the State of Pennsylvania State Police Criminal Check
to validate no criminal history. Further, all home health
aides/companions are fully bonded and insured. Accidents do
happen and if something is accidentally broken, our insurance
is in place to protect you! We tell our workers that although
they are there to do a job, they should recognize that they
are there at your request and invitation, and as such MUST
treat you, your family, and your personal property with
respect! On an on-going and yearly basis, all home health
aides/companions must complete 12 hours of continuing
inservice to insure that they stay current with trends in the
marketplace. On a weekly basis, home health aides/companions
are required to submit to Criticare/Chadds Ford Alternacare a
completed Activity Report and Timeslip. The Activity Report
indicates what services they provided. The timeslip requires a
client signature for each visit and validates the dates and
times the worker provided services. This document should
correspond appropriately with the Plan of Care initially
developed and is monitored for changes.

Criticare/Chadds Ford Alternacare provides on-going
Supervisory visits performed by a Registered Nurse. These
visits enable you to review with our Supervisory nurse the
Plan of Care and any other issues that may be of importance to
you. Also, it provides an opportunity for our Supervisory
nurse to evaluate our worker, their performance, their
effectiveness, and your satisfaction. Supervisory nurse visits
are not charged to the client and are part of our on-going
efforts to provide a quality service. Your satisfaction is
very important to us!

 The offices of Criticare/Chadds
Ford Alternacare are open Monday through Friday 9am to 5pm.
However, the service we provide is 24 hours a day, 7 days a
week. An administrative is staff available around-the-clock.
We believe this is an essential part of our commitment to
maintaining on-going and continuous communication with our
clients. Therefore, should you have a problem, question, or
staffing issue simply pick up the telephone and call! You
telephone call will be answered by a LIVE PERSON who will take
your message and telephone number. They will forward your
message to our on-call staff who will provide you with prompt
and courteous service. Additionally, at all times
Criticare/Chadds Ford Alternacare maintain an administrative
on-call person. Therefore, should an issue arise that the
on-call personnel cannot resolve you have the option of
speaking with an administrator.
